Since 2011, Live Health Foundation has delivered free preventive health screenings, evidence-based nutrition education, and chronic disease management programs to underserved neighborhoods across the greater Charlotte metropolitan area. Our mission is rooted in the belief that every individual, regardless of income or insurance status, deserves access to the knowledge and resources necessary for a long, healthy life. Through partnerships with healthcare providers, community centers, and local government, we have served over 28,000 residents with culturally competent, data-driven health programming.

Free Community Health Screening

Comprehensive preventive health screening including blood pressure measurement, blood glucose testing, cholesterol panel, BMI assessment, and a one-on-one consultation with a certified community health worker. All screenings are completely free, confidential, and available to all residents regardless of insurance status.

Evidence-Based Wellness, Accessible to Everyone

Each program is developed in collaboration with public health researchers, licensed clinicians, and community health workers to ensure culturally appropriate, scientifically validated programming that meets the needs of Charlotte's diverse population.

Flagship Program

Community Wellness Screenings

Quarterly free health screenings at community centers, houses of worship, and public recreation facilities across Mecklenburg County. Each screening includes blood pressure, blood glucose, total cholesterol, and Body Mass Index assessments. Participants receive a confidential health summary card and referral information for primary care providers accepting new patients on sliding-scale fees.

CDC-Recognized

Diabetes Prevention Workshop Series

A rigorous 16-session curriculum modeled on the CDC's National Diabetes Prevention Program. Participants learn meal planning strategies, portion control techniques, blood sugar monitoring, and stress management practices. Graduates demonstrate an average 5.2% body weight reduction and a 27% improvement in fasting glucose levels over the program duration.

12-Week Program

Heart Health Initiative

A comprehensive 12-week cardiovascular wellness program combining guided low-impact exercise sessions, weekly blood pressure monitoring, cholesterol education workshops, and stress-reduction seminars. Certified community health workers provide individualized coaching on medication adherence and heart-healthy cooking techniques, with a focus on culturally relevant dietary modifications.
